Botox and neurotoxins

Botox and similar neurotoxins are purified botulinum toxin type A injected into a facial muscle to relax it and soften the lines it makes. The US brands are Botox Cosmetic, Dysport, Xeomin, Jeuveau, Daxxify and Letybo. Neuromodulators made up about half of all minimally invasive treatments in the ASPS 2025 statistics.

The treatment

What Botox and neurotoxin changes

Botulinum toxin type A blocks the signal from nerve to muscle. It binds to motor nerve endings, enters them and cuts SNAP-25, a protein the nerve needs to release acetylcholine. The injected muscle weakens, and the lines it makes soften.

The effect wears off as the nerve forms new connections to the muscle. For frown lines between the brows, the Botox Cosmetic effect lasts about 3 to 4 months.

The same drug is used in medicine. BOTOX and BOTOX Cosmetic contain the same active ingredient in the same formulation, with different labeled uses. The first US approval was in 1989.

Each brand measures its units with its own assay. Units of one brand cannot be compared to or converted into units of another.

What it changes

  • Frown lines between the brows (glabellar lines)
  • Horizontal forehead lines
  • Crow's feet at the outer corner of the eye (lateral canthal lines)
  • Vertical neck bands from the platysma muscle
  • Lower face width from a large masseter (jaw) muscle, an off-label use

What it does not change

  • It does not restore lost volume. ASPS lists hyaluronic acid fillers for lost volume.
  • It does not lift sagging tissue. ASPS lists the facelift for sagging of the midface, jowls and neck.
  • It is not permanent. The Botox Cosmetic effect on frown lines lasts about 3 to 4 months.
  • Lines that did not soften when spread apart by hand were excluded from the pivotal frown-line trials.
  • People 65 and older responded less often than younger people in the frown-line, crow's feet and forehead-line trials.
